The Enlivio Ignite Awards celebrate the people, teams and organisations turning bold ideas into better health and care outcomes. Today, we recognise the nominees whose innovation, expertise and commitment are improving access, strengthening services and making a meaningful difference to patients and communities.

Across primary care, community services, integrated care and pharmacy, this year’s nominees show what becomes possible when healthcare expertise, purposeful technology and committed partnerships come together around the needs of patients.

Practice with a purpose award (primary care)

Dr Rasib & Partners – Transforming Access Through Digital Innovation

 

Dr Rasib & Partners demonstrates how a small primary care team can deliver outstanding patient access and compassionate care through innovation and collaboration. Using EMIS Web®, Docman and eConsult, the practice has transformed access with a structured, technology-enabled triage model that prioritises the most vulnerable patients, including palliative patients and infants, while maintaining rapid access for the wider community. The practice manages more than 53,000 patient calls annually, answering 98.7% of queued calls, and consistently achieves patient experience scores above regional and national averages. Its success has been recognised through the Marie Curie End-of-Life Practice of the Year Award and as a HTN Awards finalist. By combining proactive care, digital innovation and a strong patient-centred approach, the team has delivered measurable improvements in access, continuity and patient experience.

 

Bredbury Medical Centre – Improving Healthcare Access for Neurodivergent Patients

 

Dr Galvis has shown a persistent dedication to dismantling barriers that prevent neurodivergent individuals from accessing equitable healthcare. Through her lived experience and expertise, clinical innovation, public-speaking and resources such as the book she has co-written, she addresses significant healthcare inequalities head-on. As a GP first5 leader and through her work as a board member of B4ND, she motivates future practitioners and advocates for systemic change. Her use of technology to enhance patient accessibility and her trauma-informed approach help neurodivergent individuals receive the care they deserve.

 

Chatfield Health Care – Supporting Veterans Through Proactive Care

 

Chatfield Health Care’s veteran-focused initiative deserves recognition for embedding inclusive, proactive care into everyday practice. Through dedicated monthly GP clinics, home visits for housebound veterans, and systematic identification of Armed Forces status, the practice ensures no veteran is overlooked. Strong partnerships with Op Health and Op Courage provide seamless, holistic support for physical and mental health needs. This approach has improved access, continuity of care, and early intervention for a population that can face significant health inequalities. National recognition, including a visit from His Majesty King Charles III, reflects the initiative’s innovation, compassion, and measurable impact.

 

Muhammad Bilal Ahmed, Digital & Transformation Lead – Driving Digital Transformation in Primary Care

 

Bilal is a Digital & Transformation Lead working across a PCN serving 45,000 patients. He leads digital change across clinical and operational areas, focusing on practical solutions that improve care delivery. With a background in computer science, he has developed systems using EMIS data to improve access, continuity, and efficiency. His work supports both clinical and non-clinical teams and has delivered measurable improvements in patient care, staff workload, and service delivery. He combines technical expertise with frontline understanding to deliver sustainable improvements in general practice.

Care closer to home award (community care)

East Cheshire NHS Trust – Bringing Hospital-Level Care Home
East Cheshire NHS Trust created a virtual ward service using EMIS Web to deliver hospital-level care at home. The service has supported over 1,335 patients, improved multidisciplinary collaboration, reduced hospital admissions, and increased capacity across local services. Read the case study.

 

Blackpool Teaching Hospitals NHS Foundation Trust – Joined-Up Care with a Single Patient Record 
Implemented a single EMIS Web patient record across primary care, community and mental health services. This has improved information sharing, reduced reliance on manual communications, and enabled safer, more coordinated patient care. Read the case study.

 

Sirona Care & Health CIC – Tackling Health Inequalities
Sirona Care & Health CIC is improving health and wellbeing by tackling the wider factors that influence health outcomes, including poverty, deprivation, social isolation and access to services. Through its Community Wellbeing & Health Fund, Sirona has demonstrated how a preventative, community-based approach can reduce health inequalities, strengthen local support networks and help people live healthier, more independent lives. Focusing on priorities such as reducing health inequalities, addressing child poverty and improving access to local services, the programme directs support to those most at risk of poorer outcomes. A key strength is its collaborative approach, bringing together local authorities, voluntary organisations and community groups to provide coordinated support and earlier intervention. Digital innovation underpins this work through the use of EMIS, enabling secure information sharing across community services and GP practices, improving care coordination and supporting the early identification of vulnerable individuals. By combining community investment, partnership working and integrated digital care, Sirona has created a sustainable model that delivers care closer to home, promotes inclusion and addresses the root causes of poor health and inequality.

Better Together Award (integrated care)

Staffordshire & Stoke-on-Trent ICB – Driving Health Equity Through Integrated Care
Staffordshire & Stoke-on-Trent ICB used population health insights and collaborative working to identify and address health inequalities across communities. The programme supports more equitable access to services while improving outcomes for underserved populations. Read the case study.

 

Folkestone, Hythe and Rural PCN – Delivering Hospital-at-Home Care
Folkestone, Hythe and Rural PCN provides urgent, hospital-level care for frail and housebound patients in their own homes. The service helps avoid unnecessary hospital admissions while delivering timely, coordinated care closer to home. Read the case study.

Behind the Spark Award (partner)

Joy (trading as Pungo) – Connecting Healthcare and Community Support
Transforms social prescribing through an integrated referral platform that connects healthcare teams with over 74,000 community services. Supporting around 20.8 million patients, Joy has facilitated more than 1.8 million referrals across all 36 Integrated Care Systems. Headline numbers.

 

Inhealthcare – Scaling Virtual Wards and Remote Monitoring
Provides an integrated digital platform supporting virtual wards, remote monitoring and hospital-at-home services. Working with over 50 NHS Trusts, the platform has improved patient outcomes, increased capacity and delivered significant reductions in bed occupancy. Read the news story.

 

AccuRx – Improving Access and Communication at Scale
Delivers integrated digital triage and communication tools used by approximately 96% of GP practices in England. The platform helps reduce administrative burden, improve response times and create faster, more accessible patient care. Read the article.

 

Doccla – Enabling Virtual Care and Hospital-at-Home Services
Supports NHS organisations with remote monitoring and virtual ward services across multiple care pathways. The platform has reduced admissions, saved thousands of bed days and improved patient outcomes through proactive, technology-enabled care. Read the article.

Community Pharmacy Connect (community pharmacy)

Porthcrawl Pharmacy 
Justin and Victoria have transformed one of Wales' largest dispensing pharmacies through a strong commitment to innovation, automation and patient care. By investing in technologies such as a BD dispensing robot and embracing EPS in Wales, they have improved efficiency, safety and patient experience while creating capacity to expand clinical services. Alongside a broad NHS offering, they continue to grow private healthcare services, demonstrating how technology can support better access to care and sustainable pharmacy growth.

 

Orrell Park Pharmacy
Diana has used innovation to create a more efficient, sustainable and patient-focused pharmacy. Through the adoption of the Golden Tote, brought to market through collaboration between Phoenix and Enlivio, combining technology and supply chain innovation to give pharmacies greater capacity to focus on patient care. She improved dispensing efficiency, reduced stock holding and strengthened patient safety, creating additional capacity for healthcare services. She has successfully used these efficiencies to expand both NHS and private service offerings, demonstrating how technology can help community pharmacies improve access to care while supporting long-term sustainability.

Celebrating the people behind progress

Every Ignite Awards nominee has responded to a real health and care challenge with purpose, creativity and determination.

Together, they are improving access, tackling inequalities, connecting services, supporting earlier intervention and giving health and care professionals more time to focus on the people who need them.

Their achievements remind us that progress does not come from technology alone. It comes from people with the insight to see what could be better, the courage to rethink established ways of working and the commitment to turn ideas into lasting change.
